Position Summary

Link’s Direct Support Professionals are responsible to instruct and coach people served to promote independence in their daily living skills including in the development of social and self help skills. Plan and supervise meaningful daily activities while attending to the people’s served needs. Provide supervision and guidance to people served in all life settings.
Assure quality services that are in compliance with all accreditation and organizational standards. Provide transportation for people served. Assist in the development and ensure the implementation of assigned people served Life Plans and provide the required documentation.
Link offers Direct Support Professional positions in our Adult Day Programs – Day Habilitation and Supported Community Living –Residential Programs.
Job Description

o Instruct, supervise, and assist people served in the development of daily living skills (including but not limited to leisure, work, time and money management, good health, safety, and habit development) in accordance with their Life Plan.
o Assist people served with preparation of meals in accordance with their diets, weekly shopping for supplies, and cleaning their home.
o Work with people served to ensure residence meets agency standards of cleanliness.
o Complete necessary information for meetings, incident reports, and on all other required forms or logs as assigned with or on behalf of people served.
o Responsible to meet the medical needs of the people served and medication administration procedures.
o Plan, prepare and engage in a variety of training activities in the community, work and home settings with the people served. Ensure people served environments are comfortable, productive and stimulating.
o Assist in the development of the people's served Life Plan/CCSP as assigned, including pre and post staffing paperwork and on going progress on goals.
o Punctually and professionally attend people served meetings as assigned.
o Attend and complete necessary training in accordance with department/agency requirements to support people served in a safe and dignified manner.
o Demonstrate flexibility in work hours and locations to account for vacations and illness of other staff, including overnights. Coordinate responsibilities and coverage when absent from work.
o Represent Link Associates to internal and external stakeholders, consumers and the community in a professional manner.
o Complete and submit as instructed all required documentation on people served in a manner that is legible, timely, accurate, and contains all required information.

o Plan, monitor, evaluate and report all supported community living needs for assigned clients.
o Support client with developing goals, assessing their individual needs and working toward targeted goals
o Maintain a working knowledge of client file
o Support clients in the activities of daily living, including personal hygiene, cooking, eating, and social and leisure skills, safety skills, shopping and housekeeping.
o Support clients with health care plans, train clients on good health care techniques
o Administer medications to clients under clients’ physician orders and support clients with developing independent methods to administer medications as needed
o Actively participate in individualized and group programming, providing one-on-one support for clients as necessary to achieve goal skills.
o Support clients with maintaining a clean, safe and healthy environment.
o Support clients in searching for independent housing, and/or roommates
o Support clients with transportation issues including providing transportation.
o Monitor and document clients’ progression toward outcomes and goal achievement.
o Maintain active communication with internal team members and clients’ support team
o Provide referral and establish linkages with community agencies and services as needed by client.
o Complete all documentation as required (i.e. case notes, quarterly reports, etc.)
o Meet and maintain productivity and documentation standards
o Track and maintain position required certifications and agency required trainings

o AA Degree in Human Services or a related field with one (1) year related experience preferred. Minimum of a High School Diploma or GED. Exceptions must receive prior approval from the Executive Director.
o Must meet agency policy on transportation, background checks, and for employees in the HCBS program, this includes the requirement to use your own personal vehicle for transporting up to 3 consumers and providing on-going proof of personal vehicle insurance.
o Must meet agency Medication Manager Certification.
o Possess ability to work on a daily basis utilizing standard home, recreational and office equipment.
o Proven proficient ability to implement agency rules and procedures, use verbal, presentation, and written communication skills, human relation skills, team working skills, and organizational skills.
o Position will contain Medium physical work consisting of occasional 2-handlifting from floor to waist of up to fifty (50) pounds and occasional 2-hand push/pull of up to forty (40) pounds of force. This position consists of frequent standing, walking and reaching. The position requires frequent forward bending or crouching or stooping. The applicant will need to occasionally climb stairs without the use of railings and occasionally sit.

o Demonstrate good communication with consumers, supervisors, peers, outsider sources and family/guardians.
o Demonstrate good attendance.

o Punctually, attend all assigned meetings, in-services and trainings.
o Demonstrate involvement as a team member.

o Adhere to all applicable regulations, laws, policies, procedures and know and understand your job description.

o Submit paperwork and meet job responsibility deadlines as assigned.

o Transport consumers with agency or own vehicle as assigned.

o Maintain all required approvals/licensures/certifications.

o Perform duties in a safe manner, in order to prevent injury to themselves and others.
o Participates in and sees their role in community relations and fund raising efforts.
